What happened at CardioVascular Health Clinic?

According to a notice on the clinic’s website, between February 18, 2025 and March 4, 2025, CHC experienced a data breach incident involving their cloud services provider. As a result, an unauthorized individual may have accessed certain information stored on CHC’s network.

The incident was discovered after a network disruption on March 4, 2025, which started an investigation involving a third-party cybersecurity team. The number of impacted individuals has not been disclosed and the nature and scope are still being clarified. The information potentially exposed at CardioVascular Health Clinic, however, includes a range of personally identifiable information (PII) and protected health information (PHI), including the following:

  • Name, address, phone number, email address, date of birth, Social Security Number, driver’s license or state ID number, and financial account information, treatment and diagnosis details, prescription information, provider name, medical record or case number, Medicare or Medicaid ID number, health insurance information, and treatment cost.
