WHAT HAPPENED?

According to security incident notice letters sent out to impacted individuals, in October 2021 Family Practice Center discovered a data breach in which the sensitive personal identifiable information and protected health information in its system may have been compromised. Only in July 2022 has Family Practice begun to notify patients whose information may have been impacted.
