What Happened at Chord Specialty Dental Partners?

Chord Specialty Dental Partners (CDHA Management and Spark DSO), a Nashville, Tennessee-based dental service organization, recently announced that it experienced a data breach in which sensitive personal identifiable information in its care may have been accessed. According to the company’s breach notice, on September 11, 2024, they discovered suspicious activity related to an employee’s email account.

An investigation concluded that sensitive personal information in its systems may have been compromised by an unauthorized third party between August 19, 2024, and September 25, 2024.

The type of information potentially compromised includes the following:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Bank account information
  • Payment card information
  • Medical information
  • Health insurance information

On March 14, 2025, Chord posted a notice of the data security incident on its website and says they will soon begin sending breach notification letters to impacted individuals. Chord’s partner practices include Spark Orthodontics, Children’s Dental Health, Pediatric Dental Associates, Dentistry for Children, Children’s Dental Surgery, and Cumberland Pediatric Dentistry & Orthodontics.
