What Happened at Central Kentucky Radiology?
According to sample data breach notification letters disclosed to the Vermont and New Hampshire Attorney Generals’ offices, the potentially compromised information could include personally identifiable information (PII) and protected health information (PHI). Breached personal data can include the following: name, address, date of birth, Social Security number, date of medical service, and service charge.
An internal investigation determined that between October 16 and October 18, 2024, an unauthorized actor accessed and acquired files from Central Kentucky Radiology. The healthcare entity began notifying impacted patients by mail on June 12, 2025.

CKR provides hospital staffing services and image interpretation for medical centers and physicians’ offices across Kentucky.
