Some of the files thought to have been accessed and removed contained both personally identifiable information (PII) and protected health information (PHI).
According to a notice posted on the hospital website, the exposed information may include the following: names, contact information, demographic details, Social Security numbers, dates of birth, medical record numbers, patient account numbers, diagnosis and treatment information, prescription details, provider names, dates of service, and health insurance information.
