CorrectCare Integrated Health, a medical claims processor, has started notifying clients that the protected health information of patients has been potentially exposed. Almost half a million individuals may be impacted.

The breach affects patients treated by Mediko, Inc. in Virginia and Health Net Federal Services (HNFS) in California. Mediko provides health care services to correctional facilities in Virginia, and HNFS works with the California Correctional Health Care Services (CCHCS)/ California Department of Corrections and Rehabilitation (CDCR). Also, over 85,000 individuals incarcerated in facilities operated by the Louisiana Department of Public Safety and Corrections may have been affected.
